Mary Ann Evans Hospice's Rapid Response service provides 24/7 emergency end-of-life care for people with life-limiting illnesses. A team of registered nurses and senior healthcare assistants visits patients at home to provide advice and support with physical symptoms, administer medicines where needed, provide emotional support, and discuss advance care planning and future wishes. The service works in partnership with South Warwickshire University NHS Foundation Trust and aims to help people remain at home and avoid unnecessary hospital admissions.
