Heart Living Independently at Home

HEART is an integrated public service designed to assist older people, disabled individuals, and vulnerable residents to live safely, securely, and independently in their own homes. The partnership consists of local council housing officers working alongside the Warwickshire County Council Occupational Therapy team. Caseworkers conduct phone and in-home evaluations to identify environmental risks—such as falling hazards, dampness, electrical issues, or heating inefficiency. The service then delivers practical intervention by providing direct funding grants or by connecting residents with approved contractors to carry out structural adaptations (like ramps, stairlifts, or wet rooms). For smaller safety risks, the service includes up to four hours of free handyman labor to carry out minor immediate improvements.
